Use form for:  

• Any exception related to any requirement from the beginning of the CARES report through then end of the Gen Ed requirements

Form Name:  Exception to College Policy
Use form for:  

• Any exception related to any requirement on the CARES report after the Gen Ed requirements

Form Name:  Late Add Form
Use form for:  adding classes beyond the registration deadline, (for extenuating circumstances ONLY).
Return completed form to:  Assistant Dean, Roark 106

Form Name:  Audit/Grade Mode Form
Use form for:  changing to audit or pass/fail mode
Return completed form to:  Assistant Dean, Roark 106

Form Name:  Independent Study Proposal Form
Use form for:  students seeking credit for individualized study and research programs. MUST BE APPROVED BY DEPARTMENT AND A FACULTY MEMBER BEFORE SUBMITTING TO ASSISTANT DEAN!!!!
Return completed and approved form to:  Assistant Dean, Roark 106

Form Name:  Foundation Scholarship Form
Use form for:  awarding and renewal of student scholarships. Must be filled out before scholarship can be awarded.
Return completed and approved form to:  Scholarship Budget Manager, then forwarded to Scholarship Office for review and approval.
